Documentation Guidelines

Individuals with physical disabilities, sensory impairments, autism spectrum disorders (ASD), or other conditions, will need reports from physicians, psychologists, or other professionals that clearly state:

• The diagnosis and diagnostic criteria or tests used
• The functional impact of the disability
• A description of current treatments, medications, assistive devices, and  services
• A description of the expected progression or stability of the impact of the disability over time
• Recommendations for accommodations
• The credentials of the diagnosing professionals

Documentation requirements vary depending on the nature of the disability. Links to the documentation guidelines for the high incidence disabilities of LD, ADHD and psychological disorders, follow:

• Psychological Disorders Documentation Guidelines

Often documentation reports are within the three-year limit, but the information in the reports does not reflect a student’s current performance or needs. The Disability History Update allows information to be updated by the student or someone who knows the student well. More than one person may complete this form and send it to us.
