General Health Counseling The staff of the Health Center are Registered Nurses. Our role is to be a resource to you in illness and in health. If you are suffering from a cold or flu, stop in the Health Center for an assessment of your symptoms. If you request, we can give you a two day supply of over the counter medications to help you symptoms. We will also refer you to a primary care provider such as a physician if your symptoms warrant that. If a referral is necessary, it is helpful if you are aware of your insurance's PPO list in Dubuque. We can do strep tests in the office for a $20.00 fee. If the test is positive, our medical advisors will arrange for you to be started on a prescription. This saves you a trip to the doctor's office. We also do pregnancy tests in the office for a $10.00 fee. If the test is positive, we can help you with referrals to a primary care provider who will confirm and counsel you further. The Health Center staff does a variety of services. Stop by with any general health issue and we will assist you with care! We are here to serve students and employees of Loras. Although these shots are not given at the Health Center. We will assist you with directions to board-certified allergists who will complete this service for you. Stop by the Health Center for assistance. Health Insurance Portability and Accountability Act (HIPAA) Confidentiality All patient visits and records kept by the Health Center are confidential. Only authorized Health Center personnel may release patient records, and then, only with written authorization from the patient. Parents of patients eighteen years and older, parents or spouses of emancipated minors, and other next of kin will not have access to the medical record without the written consent of the patient. Privacy of confidential information is a social, legal, and ethical responsibility of organizations that receive medical information. The Loras College Health Center has a policy to protect the confidentiality of patient data, whether it is electronic or printed information. The Health Center can provide old health records to students who have attended Loras. To request your information: You must provide a written request to the Health Center to have your immunization record mailed or faxed. Please include your name, date of birth, social security number and signature. This request may be mailed to the Loras College Health Center, 1450 Alta Vista, Dubuque, IA 52001. You may also submit your request by fax to 563-588-7659. Please include your address or fax number or the school/employer address or fax number that you request receive the record. |
